Steps to Apply for ISH Memebrship


Steps to Apply for Membership

  1. Scan the QR Code
    Scan the official membership payment QR code using any UPI-enabled payment application (Google Pay, PhonePe, Paytm, BHIM, etc.). or Bank transfer , NEFT- Bank name :  IndusInd BankAccount No.: 150008032021IFSC Code: INDB0000568Bank Branch: Chowk-Lucknow BranchBank Address: LIC Building, Khun Khun Ji Road, Chowk, Lucknow-226003, UP

  2. Make Membership Payment  of Rs 5000 +  18% GST = Rs 5900 
    Complete the payment for the applicable membership category.

  3. Save Payment Screenshot
    After successful payment, kindly save the payment confirmation screenshot or transaction receipt for upload/reference.

  4. Fill Membership Registration Form
    Complete the online membership application form with the following details:

    • Full Name

    • Qualification & Designation

    • Institution / Organization

    • Email ID

    • Mobile Number

    • Address

    • Membership Category

    • UPI Transaction ID / Payment Screenshot Upload

  5. Submit the Application
    Carefully review all details and submit the form successfully.

  6. Application Verification
    Your membership application and payment details will be reviewed by the Membership Committee.

  7. Confirmation Email
    Upon successful verification, an official membership confirmation email will be sent to your registered email address within 5 working days.

Important Instructions

  • Please ensure that the payment transaction ID is entered correctly.

  • Incomplete forms without payment proof may not be processed.

  • Kindly use an active email ID for future communication and membership certificate ISH

Steps to Apply for Fellowship – Indian Society of Hypertension

  1. Fill the Fellowship Application Form
    Complete the official ISH Fellowship application form with all required academic, professional, and research details.

  2. Application Review & Approval
    Submitted applications will be evaluated and reviewed by the Fellowship Committee of the Indian Society of Hypertension.

  3. Receive Approval Confirmation Email
    Eligible applicants approved by the committee will receive an official confirmation email regarding fellowship approval and further instructions.

  4. Pay the Prescribed Fellowship Fees of Rs 10000 + 18% GST = Rs 11800/- 
    After approval, candidates are required to submit the fellowship fee through:

    • QR Code Payment (UPI) -

    Share the transaction ID or payment screenshot through the provided submission link/email for verification.

  6. Receive Fellowship Award Confirmation
    Upon successful fee verification, an official Fellowship Award confirmation email and fellowship credentials will be issued.

  7. Fellowship Presentation Ceremony
    The Fellowship Award will be formally presented during the Annual Conference of the Indian Society of Hypertension in the presence of distinguished faculty and delegates.
